Incident Reporting Procedures
The following actions should be performed after the incident is over.
- Within 48 hours after the occurrence of an accident that is fatal to one or more employees or results in hospitalization of three or more employees, the employer must report the accident either orally or in writing to the nearest office of the U.S. Department of Labor's Occupational Safety and Health Administration (OSHA).
- The report must relate the circumstances of the accident, the number of fatalities, and the extent of any injuries.
- OSHA may require additional reports, in writing or otherwise, concerning the accident.
- An investigation should be conducted to identify the causal factors of an incident so that the root causes can be determined and corrective actions can be taken to prevent recurrence.
- Lessons learned should be communicated as appropriate.
- There may be other local incident reporting requirements as well.
Hydrogen safety incidents and near misses should be reported in the U.S. Department of Energy's Hydrogen Lessons Learned Reporting Database at http://h2tools.org/lessons/. Please share your incidents and near misses with others who are working with hydrogen so they can benefit from your experiences. All incident reports will be "sanitized" to ensure that individuals and organizations are not identified.
